Referring doctors please send referrals by fax to
(780)-461-5442 along with relevant supporting documents.
*For General Surgery, please fax the referral to the Edmonton Zone FAST program at 780 670-3224. If you specify a request for one of our Surgeons, it will be forwarded to our office to book.
Due to the need to fully utilize valuable healthcare resources, we have a cancellation policy in place.
*4 business days notice are required for cancellations of consultations and follow up appointments.
*7 business days notice are required for cancellation of surgery or endoscopy.
